7 Days Family Dental Membership Plan Terms and Conditions

The annual membership fee for the 7 Days Family Dental Membership Plan is $309. Members receive 20% off all additional dental treatments, excluding implants and orthodontic treatments. Family members must reside at the same address to qualify for the family discount on the membership plan. Proof of residency may be required. The membership plan automatically renews annually unless canceled in writing at least 30 days prior to the renewal date. Members may cancel their membership at any time by providing written notice to 7 Days Family Dental. Any unused portion of the membership fee will not be refunded. The membership plan does not cover insurance deductibles, co-payments, or out-of-pocket maximums. The membership plan is not insurance and does not replace dental insurance. 7 Days Family Dental reserves the right to modify or terminate the membership plan at any time, with reasonable notice to members. These terms and conditions are governed by and construed in accordance with the laws of the State of Indiana. Any disputes arising under or in connection with this membership plan shall be resolved exclusively by arbitration in accordance with the rules of the American Arbitration Association. These terms and conditions are effective as of 01/01/2023.
